Investigation report: Fatal amphibious aircraft accident on Simpson Lake, British Columbia

The Transportation Safety Board of Canada (TSB) released its investigation report (A23P0063) into the June 25, 2023 fatal accident involving a privately registered Savannah advanced ultralight aircraft, that collided with the water near the shore of Simpson Lake, British Columbia.

The TSB conducted a limited-scope, class 4 investigation into this occurrence to advance transportation safety through greater awareness of potential safety issues. See the Policy on Occurrence Classification for more information.


The TSB is an independent agency that investigates air, marine, pipeline, and rail transportation occurrences. Its sole aim is the advancement of transportation safety. It is not the function of the Board to assign fault or determine civil or criminal liability.
