Marine transportation safety investigation M19C0403

The occurrence

On 15 December 2019, the bulk carrier Tecumseh had a fire in the engine room while transiting the Detroit River off Windsor, Ontario. There were 16 crew members on board at the time. The vessel dropped both anchors, and the fixed fire suppression system was used to extinguish the fire. The fire later re-ignited, and the vessel was towed to the Port of Windsor, where the fire was extinguished on 16 December with the assistance of shore-based resources.

The occurrence

On 29 January 2020, the fishing vessel Newfoundland Lynx reported a fire in the sauna. The fishing vessel Sivuliq, two helicopters, and the Canadian Coast Guard vessels Terry Fox and Henry Larsen were tasked to assist. The vessel's crew managed to extinguish the fire. The fishing vessel Sivuliq escorted the vessel to St. Anthony, Newfoundland and Labrador, where the local fire department went onboard to confirm the fire was out.

The occurrence

On , 3 of the Manitoulin’s crew members were crossing over a submerged mooring line in the vessel’s workboat while proceeding to shore near Sombra, Ontario, when tension came on the line and it struck the workboat. The impact caused all of the crew members to fall overboard. One of the crew members swam to shore and the other 2 re‑boarded the workboat. No injuries were reported.
