Marine transportation safety investigation M22C0338

The occurrence

On , the oil and chemical tanker Kivalliq W. was transiting toward Baker Lake, Nunavut, and reported making bottom contact with an uncharted object while exiting the Chesterfield Narrows. As a result, the vessel sustained damage on the starboard side. The vessel was cleared to complete its voyage on its own and proceeded to Les Méchins, Quebec, to dry-dock.

No pollution was reported.

The occurrence

On 18 June 2023, the manned submersible commonly referred to as Titan, with five people on board, was diving to the Titanic wreck site 372 nautical miles south-southeast of Cape Race, Newfoundland and Labrador. The Canadian-flagged cargo vessel Polar Prince that had been used to transport the submersible to the site was providing surface support to the submersible as it dove.

The occurrence

On 23 March 2023, the roll-on/roll-off ferry Atlantic Vision was unmooring in preparation to enter the dry dock at Les Méchins, Quebec. During the unmooring operations, one of the mooring lines was released from the dock into the water to be winched back onto the vessel. The line was drawn into one of the vessel’s bow thrusters and wrapped around the rotating thruster propeller shaft, causing the line to rapidly pull through the mooring winch on the forward mooring deck.

The occurrence

On , the passenger ferry Svanoy and a pleasure craft collided near the ferry dock in Saint-Joseph-de-la-Rive, Quebec. The pleasure craft was substantially damaged and took on water. The four occupants on board were rescued by the ferry crew before the pleasure craft sank and were transported to the hospital. Three of them sustained minor injuries and one was seriously injured. There was minor damage to the ferry. The TSB is investigating.

The occurrence

On , a crew member on board the bulk carrier Golden Zhejiang was injured following the accidental release of a free-fall lifeboat while the vessel was anchored in the Trincomali Channel, British Columbia.

The Canadian Coast Guard and the Royal Canadian Marine Search and Rescue provided assistance. The injured crew member was transported to hospital.

The TSB is investigating.
