Rail transportation safety investigation R13W0257

The occurrence

On , at approximately 1727 Central Standard Time, Canadian National freight train G84042-09, travelling eastward on the Fort Frances Subdivision, derailed 40 loaded grain cars at Mile 73.6, near Nickel Lake, Ontario. There were 2 separate groups of derailed cars, 12 cars in one group and 28 cars in the other. Several of the derailed cars ruptured, spilling grain. There were no injuries.

The occurrence

On 28 April 2013, at approximately 1840 Central Standard Time, VIA Rail northward passenger train No. P69341-28 encountered a roadbed slump at Mile 83.55 of the Canadian National Railway Togo Subdivision, near Togo, Saskatchewan. The two locomotives and the two leading cars derailed. The fuel tanks on both locomotives ruptured, leaking diesel fuel that ignited and burned until it dissipated into the subgrade material. There were no injuries.

The occurrence

On 02 May 2013, at about 1948 Eastern Daylight Time, VIA Rail Canada Inc. passenger train No. 24, while carrying out a reverse movement between the Gare du Palais and the Limoilou Yard, collided with a tractor-trailer at the private level crossing leading to the White Birch Paper mill. The last passenger car was damaged, and the tractor-trailer was destroyed. No one was injured as a result of the accident. Nearly 300 litres of diesel fuel from the tractor-trailer's fuel tank spilled on the ground.

The occurrence

On 11 January 2013, at approximately 0018, Eastern Standard Time, freight train FCN 05 collided with the rear end of iron ore train BNL 005 at Mile 124.2 on the Quebec North Shore and Labrador Railway's Wacouna Subdivision, near Mai, Quebec. The first locomotive on train FCN 05 was completely destroyed and the second locomotive derailed. Eight cars on train BNL 005 derailed. The members of the FCN 05 train crew sustained minor injuries. Approximately 40 feet of track was damaged.

The occurrence

On 24 January 2013 at 0856 Central Standard Time, eastbound Canadian National train L51141-23 struck a road grader that had stopped on the public grade crossing at Mile 33.70 of the Blackfoot Subdivision. As a result of the collision, the road grader was heavily damaged and the grader operator sustained fatal injuries. The train crew were not injured. Thelocomotive and 16 tank cars carrying dangerous goods derailed. Approximately 106,000 litres of crude oil was released from 4 cars.
