The occurrence
On , an airBaltic Airbus A220-300 was conducting a passenger flight from Riga, Latvia, to Copenhagen, Denmark. As the aircraft began the descent into Copenhagen/Kastrup Airport, the thrust levers were reduced to the idle position and the autothrottle became disengaged. The R THROTTLE FAIL master caution light illuminated shortly after the autothrottle disengaged.

On 25 July 2021, at 0656 Eastern Daylight Time, the Robinson R44 helicopter (registration C-FVPA, serial number 0846), operated by Apex Helicopters Inc., departed from a field on the outskirts of Brantford, Ontario, on a visual flight rules flight for the purpose of agricultural spraying.

The occurrence
On at 1530 Eastern Daylight Time, a privately registered, float-equipped Piper PA-18S-150 aircraft took off from Rivière du Lièvre in Ferme-Neuve, Quebec, bound for Parent, Quebec, with a pilot and a passenger on board.
During takeoff, the pilot lost control of the aircraft and it overturned in the river. The aircraft sustained significant damage, and both occupants were fatally injured. The emergency locator transmitter (ELT) did not activate.
